This book is a concise, easy-to-read manual for administrators and clinical specialists designed to help organizations and their staff meet or exceed Joint Commission and other regulatory requirements in pain and symptom management. The manual is organized in a three-ring binder with sectional tabs for quick reference. A CD-ROM is included for computerized administrative access across an organization's information systems network. One complete set of clinicians' assessment, treatment planning, and communication tools comes with each “Integrating Joint Commission Pain Standards” handbook. Additional tools for each patient or member of a facility's interdisciplinary pain management team may be purchased separately or in kits. Four durable, easy-to-use guides are included in each “Clinician's Pain Vital Sign Tool Kit.” The clinician's tools are laminated and sized to fit into a pocket or bag. Each is designed to be used interactively with patients and their families to measure pain on a regularly scheduled basis. The objective is to help clinicians record pain as the “fifth vital sign” along with blood pressure, pulse, respirations, and temperature.

Scales use a combination of progressive colors, verbal descriptions, expressive faces, and numeric ratings to enable patients to indicate their degree of pain clearly. One clinician panel provides a step-by-step guide for conducting pain/symptom severity assessments and treatment planning consistently and effectively. The other clinician panel charts specific behaviors to observe when rating pain in non-verbal or unresponsive patients.

One side provides a concise telephone script that prompts the clear flow of information between assessing nurses and treating MDs, NPs or PAs, as well as a report line that tracks patient pain, treatment, and response information exchanged between members of the interdisciplinary team.
